Electronic Case Report Form Templates


The Demographics eCRF template includes basic information about the patient, visit, and whether or not the patient meets inclusion criteria. The application checks the site number against a list of valid sites; only these values are accepted. The site number, visit date, visit number, patient ID and investigator ID values are stored so that these fields can be automatically filled in on the other templates. If the operator selects Male, the cursor skips the Pregnant and Birth Control fields.
